What information do we need?
We need your contact details – your: name, phone number, address, postcode and email address. We will also need information about your age and health in order for a treatment to go ahead.
Why do we need it, how will we use it and how long will we retain it?
We need all of the details above in order to ensure that it is safe for us to provide the treatments you may ask for and also for insurance purposes. We will store your information for 7 years after the date of your treatment, or for 7 years after you have reached the age of 18 if you are/were under 18 when your treatment took place. Our lawful basis for processing this personal information is ‘contract’ & ‘consent’ This means that we have a legal reason to ask you for your data because we need it for contractual reasons. You do not have to share your personal data with us, but if you do not we cannot offer you a treatment with Euphoria You must fulfil your side of the contract (share your personal information) in order for us to fulfil ours (carry out treatment).
At your initial visit we ask you to complete, sign & date a consultation card which then contains all of the information explained above. A member of the team will discuss this with you and they will also sign it. You are also asked if you would or would not like us to contact you in the future. If you choose not to be contacted you may still receive treatments from the euphoria team. If you have expressed a wish to be contacted we would do so in the following ways:
We send you an email approximately 48 hours prior to your appointment as a reminder. If at any time you decide you no longer require a reminder then please contact us on 0115 922 0304 or email us at firstname.lastname@example.org and we will remove that option from your details on our systems.
Follow up courtesy message
We send you an email after your appointment as a courtesy to thank you for your business. Again, if at any time you decide you no longer require a reminder then please contact us on 0115 922 0304 or email us at email@example.com and we will remove that option from your details on our systems.
We send out newsletters approximately every month with information regarding new treatments, salon news and special offers which may be of interest to you. At the bottom of every email is an ‘unsubscribe’ button which will immediately remove you from receiving newsletters.
Occasionally we may need to contact you with regard to your appointment eg If your therapist is ill and we need to rearrange your appointment we will need to contact you via the telephone, email or post.
You may wish to receive all of the above or just participate in one or two methods of contact.
